If I were to ask for feedback, this is what I would ask.

How do you feel about how I've listened to you? Tried to understand you?

How do you feel about my level of skill? Have I done anything that caused you to question my skill? Have I done anything that has helped you trust my skills more? What do you see as my strongest abilities? What do you see as my weakest abilities?

How do you feel about my level of trustworthiness in terms of intent? What do you believe my intentions are? Have I done anything that caused you to question my intent? Have I done anything that has helped you trust my intentions more?

How have I changed since I've first met you? How do you feel about those changes?
